Giel Nijpels is a Professor at VU University Medical Center in Amsterdam and a founder of Health Center Hoorn. Giel studied medicine at the University of Amsterdam and specialised as a general practitioner. In 1989, he was one of the initiators of the Hoorn Study. In 1996, he started the Diabetes Care System West-Friesland, one of the first diabetes management models in the Netherlands. Giel is also a professor of General Practice with special attention to Diabetes Care at the Department of General Practice VU University Medical Center Amsterdam. He is project leader of several scientific projects on the epidemiology and pathophysiology of type 2 diabetes, projects on diabetes prevention and diabetes care programmes, and projects on integrated care for home-living elderly people. With respect of research projects related to the epidemiology and pathophysiology of type 2 diabetes collaboration exists within the DIRECT and Rhapsody projects (IMI funded projects), the DIAMANT Study, and as coordinator of EU funded projects SPOTLIGHT and SUSTAIN (FP7 and Horizon 2020). Giel's presentation at Prague International Health Summit 2018 will describe the transition of diabetes care from centrally managed care to personalized medicine.
